Why does my mountain bike tube keep getting a flat right by the stem?

Every time I go riding uphill, my tube and tire slide on my rim, causing the stem to become misaligned with the hole in the rim. On steep parts when I am pedaling hard, the tube and tire "slip" and I can see the stem sticking out of my rim SIDEWAYS. It's no longer coming straight out. For the second straight day, this caused me to get a flat right near the stem.

The first tube was a schrader and the second tube was a presta, so the type of valve isn't the issue. I also noticed that the little rubber piece that goes between the rim and tube can easily slide around the rim.

So....has this ever happened to any of you? What did you do to fix this problem?

You need to replace the rim tape on the inside part of your wheel.

Go to your local bike shop and ask for a non slip rim tape, it should solve your problem.

suggest the following:
1st change the rim tape to a Cloth type of tape (sticky on one side). tape all the way around the rim(over the spokes) and keep it tight around the Valve-hole.
2nd Keep the pressure up in the tube. Most MTB Tyres are rated for up to 80psi, (I usually keep it near 60-65 for a little more traction)
3rd Use the nut that comes with the tube to keep the tube tight to the rim-hole

the tire is the problem its not gripping the wheel rim causing it to slide try more air pressure or new tire
